Children’s Long-Term Support
Consumer Direct Care Network Wisconsin is a statewide provider of Children’s Long-Term Support (CLTS) financial management services (FMS). The CLTS program makes Medicaid funding available for children and their families who need supports and services. Families may choose to self-direct services through county-based children’s services. We work together with counties to provide home care support.
Family Care
Family care is a long-term care option that offers comprehensive, yet flexible support for adults with disabilities and the elderly. Consumer Direct Care Network Wisconsin serves as a long-term care provider for several managed care organizations throughout the state. Family care offers the option to self-direct services and supports. As a provider, our job is to provide the right tools for success in managing supports and achieving goals.
Support Broker Services
Consumer Direct Care Network Wisconsin is a statewide provider of Support Broker Services for the Family Care program. We offer high quality assistance to meet your needs. Our support brokers help coordinate and manage self-directed services. Our team becomes your team as we build relationships to serve you.
